Do You Qualify For Free Care?
You may be eligible to receive free medical and dental services if you meet the following requirements:
- Do not have insurance through Medicaid, Medicare, Veterans benefits, disability, or an employer
- Live or work in Central Kane County
- Cannot afford medical or dental care
- Meet income guidelines listed in the chart to the right

To help us determine your eligibility, please provide the following documentation:
Proof of Residency or Employment
(You must live or work in Central Kane County)
- A recent utility bill, official mail, or similar document
- Proof of employment in Central Kane County
- Photo ID
Proof of Income
Please provide one or more of the following:
- Most recent tax return
- Pay stubs or unemployment records from the past 30 days
- A signed statement from your employer if you’re paid in cash
